PRIMARY CARE
The Base and Access Options offer primary care benefits to members who need affordable access to entry-level cover. They are based on a network of providers - Momentum Health has negotiated set rates with various hospitals and specific providers for chronic benefits and certain day-to-day benefits (such as medicine, GP visits, conservative dentistry, basic radiology and basic pathology).
NEW GENERATION
The Custom, Incentive and Extender Options offer fee-for-service benefits, where you can choose your healthcare providers for hospitalisation, chronic and day-today benefits. You control your healthcare expenses, and can receive a direct discount, depending on which provider you choose.
TRADITIONAL
The Summit Option is a traditional option that offers in- and out-of-hospital benefits payable by the scheme. No Savings and provides amounts per treatment. Ideal for members who need unlimited private hospitalisation, comprehensive chronic benefits and extensive day-to-day cover (certain sub-limits apply).

Enjoy discounts for selecting to use doctors, hospitals and pharmacies on Momentum's Associated list of preferred providers
No overall annual in-hospital limit for most plans!
Hospital accounts and related costs incurred in hospital (from admission to discharge) are covered!
50 Specialised procedures (which can be performed in a doctor's room, registered day clinic or out-patient facility), are paid!
7-days' take-home medication on discharge from hospital.
In- and out-of-hospital MRI and CT scans (co-payments apply) are paid!
Unlimited Chronic cover (according to the Chronic Disease List) for chronic conditions and serious ailments that need ongoing intervention!
Allocated "saving" funds are used to pay out of hospital costs, with some plans offering a safety net if funds are used!
International cover while traveling abroad on the Custom, Incentive, Extender and Summit Options - R5 m for hospital emergencies per beneficiary per 90-day journey (including R10 000 for dentistry, R10 000 for optometry and R500 000 terrorism cover). A R1 000 co-payment applies to all out-patient claims.
The Heath Platform. Unique benefit available to all members, FREE! It encourages health awareness, enhances quality of life and gives peace of mind. It helps you to detect illnesses early and saves you money. Subject to pre-notification you have:
Baby immunisations - all compulsory vaccinations required by the Department of Health are paid for from the Health Platform for children up to the age of 6 years.
Early detection tests - procedures such as pap smears, mammograms, DEXA scans, prostate tests, cholesterol tests and blood sugar tests. Medical examinations based on the member's age are also included.
Dental examinations - all members of the family are entitled to one dental examination per year.
Maternity programme - up to 12 antenatal visits, 2 pregnancy scans, as well as 2 paediatric visits in the child's first year.
Disease management programmes - conditions such as Diabetes, Hypertension, HIV/Aids, Oncology, Chronic Renal Failure, and Organ transplants can be effectively managed through various programmes that have been put in place to assist members with these conditions.
This is a valuable benefit that could save the average family up to R15,000 per year! And if you have a baby, up to R 20,000!

Associated hospitals are Momentum network hospitals that you may choose from when electing either the Custom, Incentive or Extender Options. They include all Life Health Care and selected Medi-Clinic hospitals. If you use a non-associated hospital, a co-payment of 30% on all claims, except in the case of emergencies will be due by you.
